First Visit

Thank you for choosing our office for your dental needs.  We have created a comfortable environment where you can feel confident in the level of dental care you receive.  When you arrive for your first visit, please be prepared to complete all health information and insurance forms which will allow us to begin your dental treatment.  To save you time during your first visit, we encourage you to complete these forms in advance by clicking HERE or on the patient registration tab.   

Your initial appointment will take approximately one hour and consist of a dental cleaning performed by one of our hygienists, 4 digital bitewing x-rays, periodontal charting to evaluate your gums, and an exam and consultation with Dr. Coffey.  You will receive a treatment plan explaining your options if there are any necessary treatments diagnosed by Dr. Coffey.

If your first visit is for a consultation with Dr. Coffey, he will perform a visual exam and review any current x-rays you may have.  You will be presented with a treatment plan and usually, treatment can be done or started the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable.
  • A list of medications you are presently taking.
  • If you have dental insurance, bring your insurance card. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office.  If they are digital x-rays, they can be e-mailed to us at [email protected].  If there is not enough time, or if they are not digital x-rays, please pick them up and bring them with you to our office. If additional x-rays are necessary, they can be taken at our office.
